We at Tiny Tennis believe your child’s safety and well being are of primary importance. We take the Children’s Online Privacy Protection Act (“COPPA”) extremely seriously and at this point, we do not feel comfortable collecting ANY of your child’s information online. We realize this may be a slight inconvenience at this time.
The enrollment process is a one time per year occurrence.
The “Enrollment Form”, “Policies Page” and “Consent to Participate Liability, Waiver and Indemnification Agreement” (which can be either downloaded and printed off from our website or can be picked up at your child’s preschool or childcare center) must be completely filled out and turned into your child’s preschool or childcare center in a designated box labeled “Tiny Tennis”. Or you may mail it to:
Mark Lopez-Director
C/O Tiny Tennis
10607 Alderbrook Pl
Louisville, KY 40299
Note: Emails or scanning of enrollment, policies and waiver documents to Tiny Tennis will NOT be accepted.
Once we have collected these three documents we will notify you immediately via email or telephone that we have received them and your child is ready to begin participating in our Tiny Tennis Program once tuition is received.
You may make a secure payment on our website immediately…even before we receive your initial enrollment, policies and waiver documents. Simply select your child’s preschool or childcare center and enter in your tuition information. At this time we do not have separate logins, so for future sessions simply select your child’s preschool or childcare center…repeat each time to enter tuition.
NOTE: However, please be aware that if we do not have the enrollment, policies and waiver documents in our possession before the start of the first class your child will not be permitted to participate until the following class. We will match up your payment with your child based on your enrollment, policies and waiver documents.
